Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (804) 741-0440 during regular business hours.
Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.
Welcome to OB-Gyn Associates! We’re glad you’ve made the decision to visit us. The information below may answer any questions you have before your appointment with us, but please feel free to call us if you think of anything else that we have not answered here.
What to bring:
Besides your Identification and insurance information, be sure to bring a list containing all of the medications you are currently taking, along with the dosage for each medicine. If you have a list of questions for your doctor, this may be helpful to have them in written form.
When you come to OB-Gyn Associates for your appointment, you may park in several places, as shown on this map. The RED DECK is the closest to our offices. If it is full, you may park in the BLUE DECK or some other areas on our campus. If you are coming to the Emergency Room, follow the signs directly there, where you may also want to use Valet Parking. When to arrive:
It’s best to arrive about 20 minutes before your appointment time to allow for parking and check-in.
What to expect:
Once you check-in at the front desk, you may be given some forms to fill out before the appointment with your doctor. Have a seat in our comfortable waiting room. Be sure to turn OFF any cell phones or other electronic items that may disturb other patients. Once you finish filling out your forms, place your papers into the Wall Pocket outside your doctor’s office area. This lets the nurse know that you are finished filling them out.
New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
We recommend that you call your insurance company to verify that we participate with your plan.
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.
To request medical records be transferred to us please contact your current care provider. It is helpful if we are able to receive those records before your appointment.

Cancellations or Rescheduling
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

After-Hours & Emergencies
